Fill out the Baptism Registration Packet and Contact Request Form to schedule a Baptism.

Baptism Registration

Parents are responsible to gather all forms, including a copy of the child’s birth certificate, and return them together to the Parish Office with the baptism fee. All paperwork must be in the office by the close of business on the Wednesday before the 2nd Tuesday of the month to receive an invitation to the on-line class.
